In accordance with Article 20 of Law on atomic energy about “Reports on the actual state of safety of radiation job performance”, in order to make report 2013 on the national work of ensuring radiation and nuclear safety, the Vietnam Agency for Radiation and Nuclear Safety requests organizations and individuals conducting the radiation work (hereinafter abbreviated to establishments) for performing:

- Report on actual state of safety management 2013 of establishments according to the form at the enclosed appendix.

- Reports are sent to the Vietnam Agency for Radiation and Nuclear Safety before December 05, 2013 (address: the Vietnam Agency for Radiation and Nuclear Safety, 14^th floor, 113 Tran Duy Hung, Cau Giay district, Hanoi city, and send soft documents (e-file) to the email: phongcapphep@varans.vn for summarization.

To be convenient in making reports, establishments may download model file from website of the Vietnam Agency for Radiation and Nuclear Safety at address: http://varans.vn, notification box.
